Peak Vista Community Health Centers Provides Notification Of Data Security Incident -04/23/21
Colorado Springs, Colo. – Peak Vista Community Health Centers (“Peak Vista”) is providing notice of an incident that may affect the privacy of certain information.
On March 7, 2021, one of Peak Vista’s healthcare facilities was burglarized and certain computer devices were stolen. Upon discovery, Peak Vista immediately notified local law enforcement and engaged a third-party team of information technology specialists to determine what information was potentially at risk.
On March 31, 2021, following a thorough analysis, the investigation determined that two of the stolen computers contained protected health information. The type of information on the computers includes names, diagnosis information, dates of birth, phone numbers, medical record numbers, and medication lists. It does not include Social Security numbers, financial account numbers, driver’s license numbers, or any other sensitive identifiers.
Peak Vista was unable to rule out the possibility that the information could have been accessed. Therefore, in an abundance of caution, Peak Vista is notifying potentially impacted individuals of this incident.
Peak Vista continues to review its current policies and procedures related to data and facility security and will make changes if warranted. Although Peak Vista has no evidence of actual or attempted misuse of information as a result of this incident, it encourages potentially impacted individuals to monitor their account statements and explanation of benefits forms for suspicious activity and to detect errors.
Peak Vista has established a toll-free hotline to answer questions about the incident and to address related concerns. The number for the hotline is 1-855-930-6231. The hotline hours of operation are Monday through Friday from 7:00 a.m. to 4:30 p.m. Mountain Time.
