The name of the decedent: Goodroad, Sundance Rainbow   Age: 46 years

The decedent was a resident of (city/state): unknown

 

The opinions listed on the death certificate are as follows:

Cause of death:  Blunt force injuries

Manner of death:  Homicide

How the injury occurred: Pedestrian run over by a recreation vehicle

Place of injury: Community park parking lot
