The following information is in reference to a death investigated by the Clark County Medical Examiner’s Office. This investigation is related to the June 8, 2023 house fire on Lower River Road in Vancouver.

The name of the decedent: O’Neill Sr., Richard  Age: 74 years

The decedent was a resident of (city/state): Tacoma, WA

The opinions listed on the death certificate are as follows:

Cause of death:  Inhalation of products of combustion

Manner of death:  Accident

How the injury occurred: House fire

Place of injury: Other person’s residence
